   Actually, Bill Duffield was Kate's lighting guy. He fell from a lighting
truss during rehersals. Kate's sound guy can be seen in the Nationwide
_Kate Bush On Tour_. He is not identified by name, but he is a bearded
guy with an accent that sound fairly Scottish (to my American ears.)

   He specifically mentions that it was built from a wire hanger. Kate
discusses that she wanted "...something like a telephonist's headset.)
It's fairly clear that she was not mearly the first to use one, but
can be fairly called it's inventor.
